Terms & Conditions for SMS & MMS Communications

These Terms & Conditions (“Terms”) govern your use of the SMS & MMS communication channels provided by Fort Wayne Radiology Association LLC (“Fort Wayne Radiology,” “FWRadiology,” “FWR,” “we,” “us,” “our”). By opting into and using our SMS/MMS program, you agree to these Terms. Please read them carefully.
- Purpose of SMS/MMS Communications
The SMS/MMS communication channel is intended solely for use by employees of Fort Wayne Radiology. It is designed to facilitate the sharing of company updates, employment-related reminders, and other internal communications.
- Patient and Client Communication: This SMS/MMS program must not be used to communicate with patients, clients, or other external parties.
- Prohibition on PHI and PII: Under no circumstances may employees use this channel to transmit, discuss, or reference Protected Health Information (PHI) or Personally Identifiable Information (PII) of patients, clients, or others. All communications containing such information must adhere to approved secure channels and protocols.
- Consent to Receive Messages
By opting into the SMS/MMS program, you consent to receive text messages from Fort Wayne Radiology. These messages may include:
- Employment-related updates, such as scheduling reminders.
- Company announcements and important notices.
- Links to surveys, feedback forms, or other internal initiatives.
You understand that standard messaging rates may apply based on your wireless carrier’s plan.
- Opt-Out and Termination of Consent
You may opt out of receiving SMS/MMS messages at any time by replying STOP to any message you receive. After opting out, we will remove you from the SMS/MMS list promptly.
- If you opt out, you will no longer receive updates through SMS/MMS.
- You may still receive updates through other communication channels, such as email or internal portals.
- Employee Responsibilities
Employees must use the SMS/MMS program responsibly and in compliance with these Terms. By using the SMS/MMS channel, you agree that:
- You will not transmit or discuss PHI or PII via SMS/MMS.
- You will not share SMS/MMS content with unauthorized parties.
- You will promptly notify Fort Wayne Radiology if you believe your phone number is being misused or if you receive SMS/MMS messages in error.
Failure to adhere to these responsibilities may result in disciplinary action, up to and including termination of employment.
- Data Collection and Usage
We collect and use your information in accordance with our Notice of Employee SMS & MMS Privacy Policy, which includes:
- Your phone number, name, and any other information you provide when opting into the SMS/MMS program.
- Usage data, such as delivery status and response rates, to improve internal communications.
Your information will not be sold, rented, or shared with third parties for marketing purposes.
- Data Security
We take reasonable measures to protect your personal information from unauthorized access, disclosure, alteration, or destruction. However, no data transmission is completely secure. By using this SMS/MMS service, you acknowledge and accept any potential risks associated with its use.
- Updates to Terms
We may update these Terms from time to time to reflect changes in our practices, policies, or legal requirements. Updates will be communicated via email or other appropriate methods. Continued use of the SMS/MMS program after such updates constitutes your acceptance of the revised Terms.
- Limitation of Liability
Fort Wayne Radiology is not liable for any delays, delivery failures, or unauthorized access to your information caused by factors outside of our control, such as carrier issues or technical problems.
